19 June 2024 – BANGUED, ABRA. The Philippine Statistics Authority (PSA) Abra Provincial Statistical Office successfully launched the full-scale implementation of change in and/or correction of entry/ies in the demographic information of the registered person on 17 June 2024 at the PhilSys Fixed Registration Center, PSA-Abra, DZPA Bldg., Rizal St., Zone 6, Bangued, Abra. This is in accordance with Section 11 of Republic Act (RA) No. 11055, otherwise known as the Philippine Identification System (PhilSys) Act, that the registered individuals are required to update their registration information in case of change in or correction of entry/entries in the information collected and stored under the PhilSys.

 

The updating and correction services will reflect the updates and correct the errors in the PhilSys-registered records of individuals. These services highlight the commitment of PhilSys in ensuring the reliability and efficiency of the National ID system. Abreños can now avail of the updating and correction services for various personal information fields, including correcting misspelled name, rectifying incorrect date of birth, updating place of birth, confirming blood type, adjusting gender information, changing address, and updating marital status.

Updating services will be provided free of charge and available at designated PhilSys fixed registration centers across the country. The registered person is advised to personally appear at the designated PhilSys FRC in the province to process his/her request. The registered person must be in possession of his/her Philippine Identification (PhillD), ePhillD, or digital National ID. The registered person shall duly accomplish the PhilSys Form No. 2 and present the documents needed. The following identification and/or supporting documents, whichever applicable, is accepted during the process:

LIST OF IDENTIFICATION AND/OR SUPPORTING DOCUMENTS

  • First Name, Middle Name, Suffix

  • Certificate of Live Birth issued by PSA, NSO, or LCRO;

  • Report of Birth issued by PSA or PSFP.

  • First Name, Middle Name, Suffix undergone correction

  • Annotated PSA-Issued Certificate of Live Birth 

  • Last Name

  • Certificate of Live Birth issued by PSA, NSO, or LCRO;

  • Certificate of Marriage issued by PSA, NSO, or LCRO (for married women);

  • Certificate of Marriage issued by Shari’a District/ Circuit Court (for Muslim married women).

  • Last Name undergone correction

  • Annotated PSA-Issued Certificate of Live Birth or Report of Birth due to RA No. 9255 and legitimation by the subsequent marriage of parents;

  • Annotated PSA-issued Certificate of Marriage due to annulment of marriage, declaration of nullity of marriage, or judicially recognized foreign divorce, ground for divorce as listed in PD No. 1083-Code of Muslims Personal Laws of the Philippines;

  • Amended Certificate of Live Birth issued by PSA (in case of Adoption pursuant to RA No. 11642) or NSO (Administrative Order No. 1 series of 1993).

  • Sex

  • Certificate of Live Birth issued by PSA, NSO, or LCRO;

  • Report of Birth issued by PSA or PSFP;

  • Annotated PSA-Issued Certificate of Live Birth.

  • Date of Birth

  • Certificate of Live Birth issued by PSA, NSO, or LCRO;

  • Report of Birth issued by PSA or PSFP;

  • Annotated PSA-Issued Certificate of Live Birth.

  • Blood Type

  • Blood typing result.

  • Permanent/Present Address

  • Barangay Certificate or Barangay ID stating the new address;

  • Any supporting document indicating the correct/updated entry of the Permanent/ Present Address.

  • Marital Status (from Single to Married)

  • Certificate of Marriage issued by PSA, NSO, or LCRO;

  • Certificate of Marriage issued by Shari’a District/ Circuit Court (for Muslim married women);

  • Report of Birth issued by PSA or PSFP.

  • Marital Status (from Married to Single)

  • Annotated PSA-issued Certificate of Marriage due to annulment of marriage or declaration of nullity of marriage)

  • Marital Status (from Married to Divorced, including Muslims and dissolution of marriage of the members of ICCs/IPs)

  • Annotated PSA-issued Certificate of Marriage.

  • Marital Status (from Married to Widowed)

  • Certificate of Marriage issued by PSA, NSO, or LCRO;

  • Certificate of Death of the Spouse issued by PSA, NSO, or LCRO.

  • Marital Status (from “Undisclosed” to any of the following:

  • Single

  • Married

  • Widowed

  • Divorced)

  • Any supporting document showing the marital status

  • In cases of clerical/typographical error

  • PSA, NSO, or LCRO-issued Certificate of Live Birth;

  • Any government-issued valid ID.
